Whether you need occasional assistance or more regular daily care, services can be added and adjusted as needs change.

What is TMT?

TMT is designed to assist with daily tasks that make life easier and more enjoyable: from housekeeping, trash removal, and laundry services; to assistance with medication and diabetes management; to meal delivery, shower plans, daily escorts, and emergency services, TMT is here to make the the routine events of daily life streamlined and stress-free.

Overall, TMT provides a full range of personal and medical care services, so that you can enjoy the perfect balance of independence and support in the comfort of your JSL home.

Personal Care Services

Personal Care Services are available as optional add-on services for a monthly fee.

These services are not included in base rent and can be customized based on your needs. Support can be layered in gradually; whether you need occasional assistance or more regular daily care, our team will walk you through available service packages and help determine what level of support is appropriate.

Daily Living Assistance

  • Routine Housekeeping: countertops, floors, bathrooms (a more extensive housekeeping service is also available).
  • Morning & Evening Care: dressing, grooming, personal hygiene, etc.
  • Escort Assistance: daily walks, meal delivery, escort services to meals and activities, etc.
  • Wellness Checks
  • Incontinence Support
  • Laundry & Trash Removal

Medication &
Health Support

  • Physician-Ordered Services: dressing changes, injections, treatments, etc.
  • Diabetes Self-Management Support: blood sugar monitoring, insulin administrtaion assistance, medical support
  • Self-Administration of Medications: reminders and assistance with managing medical treatments

Wellness & Coordination

  • Transition Support after hospital and rehabilitation visits: our liason is here to help current JSL residents reassess their ever-evolving needs, to make measured adjustments to their care without changing the familiar aspects of their lifestyle.
  • Nurse Oversight (RN/LPN supervision)
  • Health Screenings and monitoring
  • Communication with physicians and families

Meet Your New Support Network with TMT

Transition of Care Support

We employ a staff of qualified Aides and Medical Assistants to provide hands-on care under the immedient supervision of a Site Supervisor, who schedules and monitors staff, assists with care, and alerts our Nurse to any observed health problems or reported resident concerns.

Nurse Oversight

All services and clinical care are provided under the supervision of a Nurse (RN or LPN) assigned to the facility. The Nurse is regularly available to discuss health and/or care issues with the resident’s family and physician.
